Canberra Hospital Social Worker Service
Our services are available for adults over the age of 18, as well as their partner, family members, carers and meaningful others who are affected by the illness.
Social work services include:
- psychosocial support for the adjustment to, or management of a significant health condition or discharge from hospital
- community support to help prevent social admissions to hospital
- support to improve an aged person’s self-care in the community and support for their spouse or carer
- grief and loss support for a change in health status, health related bereavement or the death of an elderly spouse
- support for reactive depression due to a health condition an as referred by your doctor.
Our services are free if you have a Medicare Card or an Asylum Seekers Card. The Community Intake team can advise you of any costs when you book.
