* Visit patients in hospital to assess needs of patient

• Maintain health records and statistics

• Act as a communicator and interpreter on behalf of clients and other health workers

• Assist in case management and follow up independently, or with other health care providers

• Provide health education to individual clients and health staff

• Provide cultural life skills education to the community

• Provide support and advocacy for patients families needing accommodation and refer clients when necessary

• Give input to planning, development, implementation, monitoring and evaluation of health care plans

• Administrative tasks
